What the Job Involves
The role involves supporting clients by monitoring their mental state and managing a caseload. You will be responsible for completing various assessments such as the Care Act Assessment and carer assessments. The role also involves preparing social circumstances reports and engaging in MHAA/MCA tasks. You will ensure key performance indicators (KPIs) are current and support carers while facilitating collaborations with various agencies within and outside the NHS and social care.

- Excellent case management abilities
- Strong communication and interpersonal skills
- Knowledge of Care Act Assessment and related procedures
- Ability to work collaboratively with multidisciplinary teams
