Who You Are

You are an experienced agency social worker capable of delivering high-quality social work practice. You have the ability to work collaboratively within a multi-disciplinary team and possess a solid understanding of mental health needs in the Rhondda Cynon Taf area. You are committed to safeguarding and promoting the wellbeing of adults and can independently travel throughout the service area.

What the Job Involves

Your role will include working as part of the Taf Ely Community Mental Health Team, focusing on individuals with mental health needs. You will be responsible for co-producing and implementing care and treatment plans, acting as a care coordinator, and conducting assessments related to mental capacity and risk. Collaboration with other agencies and updating electronic records in compliance with departmental policies will be an integral part of your duties. Your work will conform to legislative measures such as the Mental Health Measures Wales (2010) and the Social Services and Well-being (Wales) Act (2014).
