What the Job Involves
You will be responsible for supporting health and social care functions for adults and carers in Wirral, ensuring the effective implementation of the Care Act 2014. As part of a multidisciplinary team, you'll coordinate social care services, provide guidance, and support safeguarding processes. Your role includes assessing and planning support for individuals with complex needs, preventing hospital admissions, and advocating for individuals and their rights. You'll also provide supervision to newly qualified social workers and contribute to the professional development of your peers.

- Knowledge of The Care Act 2014 and related regulatory frameworks
